Can you explain the sound? Also, is it from the engine itself or from the exhaust/muffler or maybe a rattle of a loose bolt/nut cause at low revs bikes/cars/piston engined things tend to vibrate a little until the whole unit is vibrating in harmony....
Not a mechanic, but it might make it easier.

If your mechanic can't fix it after listening to it on the phone....perhaps you dialled the wrong no.? Maybe it was his brother? He may have had a minor ear infection? Ringing in the ear? Lots of reasons why your bike isn't fixed by now. Why don't you video it, put it on youtube and every mechanic in the world can have a go? Just a thought.

If it's ticking, it is usually (but not always) related to the valve train, if it's screaming it must be a reciprocating part or assembly (bearing, cam chain/tensioner, ect...). I'm sure someone local to you wil offer an ear for the cause... good luck.
